First off, in answer to the NPC questions:
NPCs can and do die, though deaths NOT due to natural causes like old age are very rare just due to advanced medical technology and free, widespread healthcare. In general, death isn't something this game is intended to focus on (so like, we're probably not going to be killing off any NPCs), but it does exist and isn't banned from mentioning. People in this world just tend to be really healthy!
Likewise, they age physically and have children and generally do everything that normal people would do!
Basically, while some of the goofier game mechanics with no logical IC explanation are definitely inspired by the video games (aka The Ledges of Doom, or player-characters who should be super dead waking up in the Pokemon Center instead of actually dying), the setting of Victory Road is otherwise inspired by the anime, where people are just people living their lives! Walking out on the street and interacting with locals shouldn't feel any less ordinary than it would for anybody visiting a new country for the first time and experiencing a different culture. Some folks definitely come off as kind of weird (ie, Youngster Joey being physically incapable of shutting up about his Rattata), but no one feels like an ageless death-defying computer simulation or anything.
NOW for the other questions!
1. Yes, when a character canon-updates, their picture and age are reflected on their trainer ID.
2. Yes, characters may find the Day-of-the-Week people on the listed routes and get the item from them! It has to be on the right day of the week though! So you could only obtain a Twistedspoon if you were on Route 29 on a Tuesday and ran into the Twistedspoon guy.
